The Short Version
If you are a paying customer (either a provider, healthcare provider or other health care professional that registered you (the "User") for an account, or if you are a User taking the Interview without a referral) then the Company collects your name, contact info, and credit card or other billing information (collectively, "User Information"). The Company uses User Information to bill you and to send you occasional communications related to your account and/or the Company's products and/or services. The Company does not sell or share User Information with others except as required by law, to do business with you (e.g. we share your credit card info with our credit card processor), or in an aggregated and/or anonymous basis for research purposes.
If you take the Indigo Mental Health Assessment™ then we “collect” all of the information you enter into the interview, including your name, contact info, and psychiatric symptoms. We use that information to calculate diagnoses per the DSM-5 diagnostic criteria.
If you took the Interview in a clinical setting or by clicking on a link sent to your email by your healthcare provider then we will send the Interview report (the "Interview Report") to your healthcare provider. This Interview Report includes all of the information you entered into the Interview, plus the calculated diagnoses (if any).
We may send a copy of the Interview Report, with your personally identifiable information removed (so that you are anonymous) to The Company for quality control and program enhancement research.
Other than your healthcare provider (if any, as described above) and yourself, we do not share your Interview Report with anyone else except as required by law. Furthermore, we do not retain any information from your Interview once you have completed the Interview.
We follow industry-standard best practices to protect your data. Data that travels over the Internet is encrypted to prevent snooping by Bad Guys. Sensitive data that we hold on to, such as passwords and payment information, are encrypted on our servers.
Types of Website Users
The Site accommodates different types of users, with vastly different privacy implications. This section describes these types of users and provides a brief overview of the privacy issues addressed for each type of user.
The Site provides an online psychiatric diagnostic tool called the Indigo Mental Health Assessment™. Healthcare providers or other mental health professionals can sign up for an account on this website. Signing up for an account involves collection of the usual e-commerce information, including but not limited to contact information (name, address, phone number, email address) and payment information (credit card etc.).
Once an account is created, the healthcare provider can use that account to allow patients to take the Indigo Mental Health Evaluation™. The Interview process collects sensitive personal and health information (but not payment information) from the patient taking the Interview and transmits that information, plus possible diagnoses, to the healthcare provider.
In addition, a User may come to the website without a healthcare provider referral and, for a fee, take the Indigo Mental Health Evaluation™ and receive the results. In this scenario, the website collects the usual e-commerce information in addition to sensitive health information. However, in this scenario the patient’s health information is not shared with a healthcare provider.
Clinicians are website Users who have signed up for an account. The website collects e-commerce information from healthcare providers.
Patients are website Users who take the Indigo Mental Health Assessment™. The website collects personal and health information from patients.
If the patient was referred to the website by a healthcare provider then, with the patient’s permission, the website may transmit that information to that healthcare provider. In this scenario, the website may collect e-commerce information from the patient if the healthcare provider has chosen a subscription plan that requires patients to pay for interviews directly.
If the patient was not referred to the website by a healthcare provider then the website will collect e-commerce information to process the patient’s payment for this service. In this scenario, we do not transmit the patient’s health information to anyone other than the patient.
Collection of Your Personal Information
The Company may collect personally identifiable information.
If you are a healthcare provider (as defined above) then we may collect personal data necessary to maintain your account, including but not limited to your name, address, phone number, email address, and professional credentials. We may also collect information that we can use to verify your professional credentials (license number, NPI number, etc.).
If you are a paying customer (either a healthcare provider, as defined above, or a patient paying to take the Indigo Mental Health Evaluation™) then we will collect the e-commerce data necessary to bill you, including but not limited to your name, address, phone number, email address, and payment information (credit card or other payment information).
If you are a patient (as defined above), The Company may collect personally identifiable information, including but not limited to your name, phone number, and email address. Since you are taking a psychiatric Interview, The Company may also collect sensitive health information. If you were referred to the Site by a healthcare provider (as defined above) then The Company may transmit the information that we collect from you, plus potential psychiatric diagnoses derived from that information, to the healthcare provider.
If you are a patient then you have a right to view and correct the Interview results. During the course of the Interview you can go back and correct earlier entries. When the Interview is complete you can view the diagnoses (if any) in the web browser. You can also request that a copy of the Interview Report be delivered to you as a password-protected email attachment. Your personal and health information which was collected by the Interview is deleted when you terminate your web session.
We may also collect anonymous demographic information, which is not unique to you, such as your age and gender. We may gather additional personal or non-personal information in the future.
Information about your computer hardware and software may be automatically collected by The Company, including but not limited to your IP address, browser type, domain names, access times, and referring website addresses. This information is used for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the Site.
The Company encourages you to review the privacy statements of websites you choose to link to from the Site so that you can understand how those websites collect, use, and share your information. The Company is not responsible for the privacy policies (or lack thereof) for other websites outside of or linking directly or indirectly to or from the Site.
Use of Your Personal Information
The Company collects and uses your personal information to operate its website(s) and delivers the Interview Reports to the healthcare provider who initiated or authorized the associated interview.
The Company may also use your personally identifiable information to inform you of other products or services available from The Company and/or its affiliates. The Company may also contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.
The Company does not sell, or license its User lists to third parties.
The Company may share data with trusted partners to help perform statistical analysis, send you email, postal mail, or other communications, provide customer support or billing, or arrange for deliveries.
The Company will disclose your personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to legal requirements or processes, (b) to protect and defend the Company's intellectual property rights or (c) act under exigent circumstances to protect, or potentially protect the personal safety of Users, individual persons, or the general public.
Most Web browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies if you prefer. If you choose to decline cookies, you may not be able to fully experience the interactive features of the Site or other websites that you visit.
Security of Your Personal Information
To secure your personal information from unauthorized access, use or disclosure, the Site uses the following services and techniques:
The User's Interview responses and Interview Report are not retained on our servers after the Indigo Mental Health Evaluation™, they are encrypted and transferred to HIPAA-compliant servers.
If a patient has been referred to the Site to take the Indigo Mental Health Assessment™ then the Interview results are encrypted prior to transmitting the results to the healthcare provider.
All personal information traveling over the Internet is transmitted using SSL encryption facilitated by a GoDaddy Extended Validation Certificate (www.godaddy.com/ssl/ssl-extended-validation.aspx).
Opt-Out & Unsubscribe
The Company respects your privacy and will give you the opportunity to opt-out of receiving announcements. Users may opt-out of receiving any or all communications from The Company by clicking the “unsubscribe” link in any applicable email from The Company, or by contacting us at firstname.lastname@example.org.
